CERTIFICATE OF INSURANCE

Each sponsor/vendor must submit a Certificate of Insurance. You can find more details and the form here.

Please review all details and email your Certificate of Insurance to Sean Sweeney at ssweeney@lifetimefitness.com by Friday, April 28, 2017.

Type of Insurance and Limits

• Commercial General Liability Coverage
• Worker’s Compensation/ Employer’s Liability Coverage
• Commercial Automobile Coverage
• Umbrella Liability Coverage (Occurrence Basis)

In addition to referencing the aforementioned types of insurance and limits, each Certificate of Insurance must also reflect the following:

• The following must be named as an Additional Insured on a primary and non-contributory basis: Life Time Fitness Inc., Life Time Media and Events, and its affiliate companies, as well as their employees, officers, directors, and agents of such companies, relative to the services provided. Life Time Fitness, Inc. 2902 Corporate Pl., Chanhassen, MN 55317

Should any of the above-described Policies be cancelled before the expiration date thereof, notice will be delivered in accordance with the Policy provisions.

Workers Compensation insurance is required even if the contractor is not required by the state to have this coverage.
